Mason Anthony School of Cosmetology Arts and Sciences offers a full-service salon/spa experience. Our discounted services are given to the public by enrolled students in the Cosmetology, Manicuring, Esthetics, and Massage Therapy Programs under direct supervision of full licensed instructors.

Please acknowledge that we are a school; therefore, our students are not fully trained or licensed in the state of Ohio. Our staff and students will strive to provide excellent care during your services.

By signing this waiver, you understand that all services must be paid in full at the scheduled appointment time. If you are unhappy with the results of your service, Mason Anthony School staff must be informed within 48 hours of your service. After staff is informed, we will offer one complimentary appointment to achieve the desired results. The complimentary appointment must be scheduled within two weeks of the first appointment.

Please acknowledge that there are inherent risks associated with receiving services from students that are not fully trained or licensed. Some risks include but are not limited to skin or scalp irritation, redness, allergic reactions to chemicals or ingredients found in products used during services.

By signing this waiver, you voluntarily, willingly, and knowingly assume all risks, known or unknown, in any way associated with receiving services from any student or employee of Mason Anthony School of Cosmetology Arts and Sciences.Mason Antony School of Cosmetology Arts and Sciences has the right to refuse service to anyone behaving inappropriately or if their state of health may influence the effects of the service.

Please acknowledge that if you are more than 20 minutes late to the scheduled appointment with no call ahead, you will be charged an additional $5.00 to the service. After 30 minutes with no call ahead your appointment will be canceled.

I do hereby release, hold harmless, and waive all claims associated with all services received, thereby releasing Mason Anthony LLC and any parent or subsidiary for all loss, liability, damage, claims, demands, injuries, and costs for myself, my heirs, personal representatives and/or assigns.
